New Patient Registration

If you are new to the Smethwick area and are requiring to register with our GP please ask at our reception. New Patients should collect a registration form for each member of the family, complete and return it to reception with a form of ID and proof of address (this will enable staff to find your medical records from a previous GP Practice). 

Please note –  if you do not have ID or proof of address you will still be able to register at Lodge Road Surgery

Following receipt of completed registration form, our registration clerk will register your details on our system, please allow 2-3 working days. If you require urgent medication whilst your registration details are being uploaded, kindly supply proof of medication to our receptionist in the form of a repeat medication slip.

When registering at the Practice you will automatically register for on-line services at the Practice, unless you state otherwise.

Overseas Visitors

Those who visit the UK from overseas must pay for their care when they are in England.

In 2015 changes were made to how the NHS charges overseas visitors for healthcare so the NHS does not lose out on income for the services these patients have received.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

We can only offer a temporary resident appointment if there are appointments available.
